Pflugerville High School New Student Registration Process

Students brand-new to Pflugerville ISD or who are returning from another district or the Provan Opportunity Center will need to complete the new student registration process. To register please provide the following:

A birth certificate

A socail security card

Proof of residency (lease agreement with signature page complete, updated utility bill, etc.)

Drivers' license or state ID wth picture of parent/guardian enrolling student (Parent or guardian must be present at the time of registration)

Last report card or transcript - for incoming 9th graders, the 8th grade report card must indicate that the student has been promoted to the 9th grade, for upcoming 10th-12th graders, a transcript is the preferred document. Transcripts aid in proper class selection as students near graduation.

TAKS results (if available - for in-coming Seniors)

Up-to-date immunization records that include:

DTaP/DTP/DT/Tdap/TD – five doses, one of which must have been received on or after the fourth birthday; however, 4 doses meet the requirement if the 4th dose was given on or after the 4th birthday.

Ages 7 years or older – Three doses, including one dose on or after the fourth birthday.

(pertussis vaccine is not required) Tdap/Td/DTP/DTaP or any combination of these

Booster Dose- (Tdap/Td) -One dose is required ten years after the last dose of DTP/DTaP/DT

Polio – Four doses of polio one of which must have been received on or after the fourth birthday; however, 3 doses meet the requirement if the 3rd dose was given on or after the 4th birthday.

Measles, Mumps, and Rubella (MMR)- Two doses of a measles-containing vaccine with the first dose on or after the first birthday; second dose required upon entry into kindergarten.

Hepatitis B – Three doses

Varicella – One dose on or after 1st birthday (Two doses if vaccine given at 13 years or older)

Proof of shot or previous illness may substitute for vaccination.

Hepatitis A –Two dose series is Highly Recommended.
