PHS Health Services/Nurses Office Information

Hours:

8:00 a.m. to 4:15 p.m. excluding a 30-minute lunch break ( 12:45 p.m. to 1:15 p.m. ) Emergencies may be referred at any time between 8:00 a.m. and 4:00 p.m.

Hall Pass:

After the first bell of the day, a pass is REQUIRED any time a student comes to the clinic. The student MUST have a pass from the teacher whose class the student is missing. Students are not seen during passing periods. Pink passes for the clinic are provided for teachers' use. Supplies of passes will be replenished upon request by handing them to the teacher or placing the passes in the teacher's mail box. Students may not pick up passes for teachers.

Emergencies:

Use the classroom intercom system. The FRONT OFFICE staff will notify the nurse/administrator, and response will be faster.

Medicines:

All medicines, prescription or over-the-counter, are given by the nurse or school employee assigned by the principal, according to PISD policy. We do not provide ANY over-the-counter preparations. By district policy, students are not to keep any medications, and are subject to discipline if they do so.

Student Calls:

Students who are sick or have a health need should be sent to the Nurse's Office to make telephone calls to their parents . Otherwise, it is often impossible to find the student if the parent needs to return the call.

Clinic Log:

Students are required to sign in to the clinic upon arrival. Documentation is kept for all clinic visits. Information is available to teachers upon request for validation of time absent from class.

Request for Student:

Probable reasons: vision and/or hearing screening, notification of an immunization needed, health need referral from a parent (including the taking of a medication), follow-up and evaluation of previously given care.

Teacher Referrals:

The nurse invites teacher conferences regarding observed or known health problems/needs of students that might interfere with learning ability or classroom performance.

Notice of Specific Need:

When a student's health need requires some special consideration or privilege, a notice will be sent to the student's scheduled teachers, principal, and counselor.

Care in the Nurse's Office

After an assessment, nursing care may include:

  1. first aid
  2. a short period of observation and rest
  3. consultation with parent
  4. use of alternative modalities of care: crackers, mint, warm or cold compresses, salt gargle, rest, heating pad, contact lens solution/storage, etc.
  5. over-the-counter medication only if sent by parent and in accordance with district policy

Outcomes include:

  1. student returns to class
  2. student returns to class with a plan to return if necessary
  3. student will go home (MUST SIGN OUT AT THE OFFICE)
  4. Health consultation/teaching based upon health need presented
